Complete managed handoff between VA and civilian facilities — in either direction. Military record translation, medication continuity, specialist introductions, and 72-hour post-transfer follow-up. No veteran should fall through the gap between two systems because no one was managing the handoff.

Advocacy for military spouses, dependents, and caregivers navigating TRICARE, civilian networks, and the unique stressors of military family life. We coordinate between providers, manage referrals, and make sure military families receive the same level of organized advocacy as any other Transcend client.

Ongoing Veteran Care Management

Monthly retainer for veterans managing complex chronic conditions — TBI, PTSD, MST-related health impacts, spinal injuries, and multi-system conditions common in combat veterans. Quarterly check-ins, lab review, medication oversight, and emergency escalation on-call.
