For situations involving a single facility and a clear, contained timeline — for example, one hospital admission with a straightforward discharge. Includes record review, family briefing, and a printed care coordination summary.
For situations involving multiple specialists or one care transition (e.g., hospital to rehab or home), with moderate documentation and coordination needs across providers who may not be communicating with each other.
For situations involving multiple specialists, multiple care transitions, ongoing escalations, and/or special circumstances such as religious care directives or legal considerations. Includes full documentation suite: care coordination tracker, lab trend analysis, escalation scripts, and discharge coordination packet.
Bill audits, error identification, insurance appeals, and payment negotiation. Up to 80% of hospital bills contain errors — we find them. You only pay the percentage fee if savings are found.
Note: Hourly Ongoing Care Management retainers will be introduced once Transcend has an established track record of client cases. For now, all navigation work is offered as flat-fee, predictable pricing.
